I face indeed the same issue, i recently started using my Pi3B+ again to make a Sega only system, and after a clean install, FBNEO performs really poorly. compared to the FBALPHA it used to run before i stopped using that Pi.
In any event I'll try the suggestions here, and also have a look which version is included by default before i update it.

I did a PR at https://github.com/libretro/fbalpha2012/pull/99, but i believe such change will have a performance cost, even on classic m68k games (additional condition on every cycle, and the code for 68ec020 looks like it'll be slower).

idk about fbalpha2012, but in fbneo those settings are available through core options.
i'm wondering though, why are you using the old fbalpha2012 instead of fbneo ? you have a very old raspberry pi or something ?

So, I know this is quite old, but I actually found a very quick solution for this.

If you go into the Retroarch settings and navigate to Settings > Video > Turn Hard GPU Sync to ON.

I was playing Double Dragon, and the framerate kicked up immediately. The game runs flawless now.
